Why register for your COVID-19 Test?
It saves you time at the testing site and ensures that all the information provided is submitted correctly.
However, please only submit the form on the day that you arrive to get tested because that is the date of service that will appear on your report.
You can register yourself and/or your dependents under the age of 16.
